Please Note: Some insurance plans require a referral in order for you to be treated by one of our providers. At the time of scheduling, please ask if you will need a referral from your primary-care physician. Most cosmetic and aesthetics procedures cannot be billed to insurance and referrals are not necessary.

To make your check-in process faster and easier, please download the registration forms on the right hand side of this page before your visit. On the day of your appointment, please also bring with you:

• Your insurance card
• Health history and medications list
• Physician Referral form, if needed

We encourage new patients who have not completed our patient registration forms to arrive 10 minutes early to fill out paperwork.

We understand that your plans may change. In order to avoid being billed, please give us at least 24 hours notice if you must cancel or reschedule your appointment.

Please bring your insurance card with you to your appointment. We accept most major insurance plans.

All co-payments, co-insurances and deductibles are due at the time of your visit. Any services that are not covered by your insurance are your responsibility. Because cosmetic services are not covered by insurance, we accept the following forms of payment: cash, credit card and check.
